● An up-to-date record of their child's immunization ● A medical exemption letter completed by a qualified medical practitioner that clearly states the medical reasons why the child cannot be immunized (if applicable). ● A letter provided in writing objecting to the immunization on the grounds that it conflicts with sincerely held convictions of a parent's conscience or religious beliefs (if applicable). Please email a copy of their immunization record to cdekoker@tfs.ca (Toronto campus) or msherifhabib@tfs.ca (West Campus) as soon as possible or bring it to the office on Orientation Day. Please note that students will not be able to attend school if their immunizations are not up-to-date. Medication – Medical forms must be completed for any ongoing medication required. Current allergy lists and other medical issues lists, indicating the child's name and allergy, are posted in Jardin d'éveil and PK classrooms. These are updated regularly to ensure information is accurate. – Medication will only be administered when a Medication Information Form has been completed and signed by the child's parent (form available at the Branch Office or from Jardin d'éveil staff). This includes products such as hand cream, sunscreen, lip balm or non medicated ointment. To view the Medication Form, click here. Anaphylaxis Policy and Procedure Anaphylaxis is a severe allergic reaction that can lead to rapid death if left untreated. The school cannot assume responsibility for providing a completely allergen-free environment. As a nut-restricted school, we have established procedures for staff, students, parents and visitors to reduce the risk of accidental exposure. The school requests that all students and adults who visit or work in the school do not bring or send nuts and nut products to school or on out-of-school activities, and avoid bringing or sending products that are labelled "may contain nuts." Alert sheets are posted in a prominent location in all teaching and commons areas and all staff are trained on the recommended emergency treatment (administration of epinephrine by an auto-injector).
